If your student is unable to participate in gym/sports due to an injury or illness, please provide a note from the treating health care professional indicating the diagnosis and length of restriction. We will also need a note from the health care professional indicating that the student is cleared to return to gym/sports. Click here for any other restrictions.
iIt is important to submit updated physical evaluations so the health office staff is aware of any health concerns. All students new to the district, from out of state OR out of country are required to submit evidence of an updated physical evaluation within 30 days of registration.
