Importance of Medical Examinations
All students should have a medical examination at least once during adolescence (Grade 7 through Grade 12) (N.J.A.C 6A:16-2.2)
New students need to provide within 30 days of enrollment entry-examination documentation.
If your student plays sports or is interested in playing sports, you will find useful information here.
If your student has an existing medical issue requiring medical intervention, medication to be administered during school hours, or has asthma or any allergies, you will find the required medical forms here.
New Jersey requires students be up to date with their immunizations. You can find more information here.
Out of Gym Policy
If your student needs to be out of gym, you must provide a doctor's note to keep the student out of gym. It must be updated by the doctor every 30 days. Also, if the student plays sports, they will not be able to return to their sport unless the doctor's note states "can return to gym and sports".
